Job summary

Behavioral Health Network, Inc. is seeking a Care Coordinator II to join our Adult Community Clinical Services (ACCS) team.

You will oversee clinical operations for an Integrated Team and Group Living Environments, ensuring high-quality, person-centered care and coordination across behavioral health, medical, and community services. The role requires a Master's in a mental health field and LPHA licensure, with at least two years in human services.

Qualifications

  • Master's degree in a mental health-related field required.
  • Current LPHA licensure (e.g., LCSW, LICSW, LMHC) required.
  • At least two years of experience in the human-services field.
  • Knowledge of assessment, treatment planning, clinical intervention, care coordination, and LPHA responsibilities.

Responsibilities

  • Oversee the clinical operations of an ACCS Integrated Team and associated Group Living Environments.
  • Complete comprehensive clinical assessments and develop individualized treatment plans.
  • Provide direct clinical interventions within LPHA scope and ensure fidelity to treatment plans.
  • Model and supervise outreach and residential staff to maintain consistent care.
  • Coordinate behavioral health, medical, substance-use, housing, and community services to support recovery and community integration.
  • Monitor progress, adjust interventions, and collaborate with leadership and providers.

What You Will Do

Oversee the clinical operations of an assigned ACCS Integrated Team and associated Group Living Environments while coordinating high-quality, person-centered care for individuals served.

Complete comprehensive clinical assessments and develop individualized treatment and recovery plans in partnership with each individual and the multidisciplinary team.

Provide direct clinical interventions and perform functions reserved for a Licensed Practitioner of the Healing Arts in accordance with ACCS contract requirements.

Provide clinical modeling, consultation, and oversight to outreach and residential direct-care staff, reinforcing consistent interventions and fidelity to each individual's treatment plan.

Coordinate behavioral health, medical, substance-use, housing, benefits, and community-based services to support recovery, stability, human rights, and successful community integration.

Monitor clinical progress, emerging risks, and changing needs; guide timely interventions and collaborate with program leadership and internal or external providers when urgent concerns arise.

Ensure treatment plans and clinical interventions reflect trauma-informed, recovery-oriented, evidence-based, and culturally responsive practices.

Maintain compliance with Massachusetts Department of Mental Health, ACCS, funding-source, contractual, clinical, and agency requirements.

Complete and review clear, accurate, timely clinical documentation and electronic health records that meet all ACCS and DMH standards.

Participate in team communication, case consultation, meetings, quality-improvement activities, and other responsibilities assigned by program leadership.

Who You Are
  • Master's degree in a mental health-related field required.
  • Current clinical licensure that meets LPHA requirements, such as LCSW, LICSW, or LMHC, required.
  • At least two years of experience in the human-services field required.
  • Knowledge of comprehensive assessment, treatment planning, clinical intervention, care coordination, and the functions and responsibilities of an LPHA.
  • Knowledge of the relationship between mental health and medical conditions and systems, substance use, care management, recovery principles, evidence-based practices, and trauma-informed treatment.
  • Commitment to individual empowerment, growth, informed choice, human rights, dignity, and successful community integration.
  • Ability to provide clinical guidance and oversight to multidisciplinary outreach and residential teams without formal personnel supervisory responsibility.
  • Strong clinical judgment, collaboration, interpersonal, verbal, and written communication skills, with the ability to work effectively with individuals, families, providers, and colleagues from diverse backgrounds.
  • Strong organization, time-management, prioritization, documentation, computer, and electronic health record skills, including the ability to respond effectively to urgent needs and frequent interruptions.
  • Ability to work across assigned Integrated Team and residential program settings and meet the physical requirements of the role, with or without reasonable accommodation.

Salary Range

Salary Range: $55,000-$72,000
