Qualifications

• High school diploma or GED.
• MAP Certification; training offered within 6 months of employment.
• CPR/First Aid; training offered within 3 months of employment.
• Valid Driver’s License with at least one year of driving experience.

Responsibilities

• Assist individuals with daily living activities including personal hygiene, bathing, dressing, feeding, grooming, transferring and meal preparation.
• Transport individuals to appointments and community activities using wheelchair vans.
• Complete Individual Support Plans (ISP) and track progress toward personal goals.
• Maintain daily logs, notes and behavioral data.
