Job summary

Qualifications

• High school diploma or GED required.
• Ability to read, write, and comprehend English to follow care plans and document services.
• MAP Certification training offered within 6 months of employment; CPR/First Aid training within 3 months.

Responsibilities

• Assist individuals with daily living activities (hygiene, feeding, dressing, grooming, mobility).
• Transport individuals using wheelchair vans to appointments and activities.
• Complete ISP to support goals and independence; maintain daily logs and behavioral data.
