Job summary

Community House Mental Health Agency in Seattle, WA is seeking a dedicated Case Manager to serve as the primary on-site contact for residents. You will coordinate service plans, assist with crisis plans, and ensure timely intake and documentation while upholding trauma-informed care principles.

The role requires coordinating care with internal teams, arranging transportation for appointments, and documenting interactions in the EMR.

Responsibilities

• Serve as secondary Case Manager and primary on-site contact for residents at assigned location; coordinate Service Plans and Crisis Plans.
• Coordinate referrals and intake into the program; review individualized plans.
• Collaborate with care team to ensure access to treatment and appointments, including transportation.
• Maintain confidentiality and adhere to trauma-informed care principles.
• Manage medication access following the seven rights and complete required documentation.
• Participate in shift change meetings and required documentation in EMR.
