Qualifications

• Master’s degree in behavioral health or social science.
• Experience serving adults with severe and persistent mental illness.
• Apply for Agency Affiliated Counselor credential within 30 days of hire, if not licensed.
• Successfully pass criminal background check before hire date.
• Complete Assisted Living Facility Training requirements (fingerprinting, Food Workers Card, First Aid/CPR, 75-Hour LTC training, Home Care Aide, CE hours) within specified timelines.

Responsibilities

• Provide direct individual mental health counseling and therapeutic interventions to residents per service plans.
• Provide clinical consultation and guidance to residential staff and non-MHP case managers.
• Contribute to licensing compliance through clinical services and documentation.
• Serve as Primary Case Manager for assigned caseload, coordinating services and maintaining plans and documentation.
• Support on-site care team with collaboration and non-supervisory role in clinical work.
• Coordinate with internal/external departments for referrals and intake.
• Collaborate with care team to ensure access to treatment, case management, and appointments.
• Ensure confidentiality and trauma-informed care in all interactions.

Education
