Qualifications

• Apply for Agency Affiliated Counselor credential with Washington Department of Health within 30 days of hire.
• Successfully pass criminal background check before hire date.
• Obtain/maintain First Aid/CPR card within 30 days of hire.
• Complete TB test within first 2 weeks of hire.
• Hold a valid license with clean driving record to use agency vehicles.
• Able to navigate and comprehend charts in electronic medical record (EMR).
• Able to communicate information clearly to others.

Responsibilities

• Serve as secondary Case Manager and primary on-site contact for residents at assigned location, coordinating needs and services.
• Coordinate with internal and external departments to coordinate resident referrals, ensure timely intake, and review plans.
• Collaborate with the care team to ensure access to treatment, support, case management, and appointments.
• Ensure all interactions follow trauma-informed care principles: safety, choice, trust, collaboration, empowerment.
• Coordinate access to medications following the seven rights and proper documentation.
• Complete daily EMR documentation of interactions, medications, and required notes.
• Practice financial stewardship of agency resources and monitor petty cash usage.
• Meet identified KPIs and participate in discharge planning and information releases as needed.
• Assist with on-site cleaning, including training residents in space maintenance and pest treatment prep.
