Job summary

Community House Mental Health Agency in Seattle is seeking a Diversion Residential Case Manager to provide behavioral health services, skills training, and community support in a residential diversion program. You will coordinate treatment plans, documentation, and care reviews for your caseload.

The role emphasizes trauma-informed care, collaboration with a multidisciplinary team, and supporting residents with access to medications and appointments.

Qualifications

• Obtain and maintain Agency Affiliated Counselor credential/licensure within 30 days of hire
• Pass criminal background check before start date
• First Aid/CPR card or certification within 30 days of hire
• TB test within first 2 weeks of hire with acceptable results
• Valid license with clean driving record to use agency vehicles
• Navigate EMR charts and communicate clearly within a team
• Able to work independently and with a team, and maintain confidentiality

Responsibilities

• Serve as secondary Case Manager and primary on-site contact for residents at assigned location, coordinating resident needs and services
• Coordinate with internal/external departments for referrals and timely intake
• Collaborate with care team to access treatment, support, and appointments including transportation
• Ensure trauma-informed care principles in all interactions (safety, choice, trust, collaboration, empowerment)
• Assist with medication access following seven rights and training
• Participate in shift change meetings and complete related documentation
• Document resident interactions and medications in EMR accurately
• Practice good financial stewardship of agency resources and monitor petty cash
• Meet key performance indicators for the program and discharge planning as needed
• Assist with on-site cleaning and activities, and support residents with ADLs/IADLs
