Job summary

Community House Mental Health Agency is seeking a dedicated Diversion Case Manager in Seattle to coordinate care for adults with mental illness. You will manage a caseload, ensure service plans, crisis plans, and documentation are up to date, and work with a multidisciplinary team to meet client goals.

The role requires a Bachelor's degree in social science or related field, with 2+ years of relevant experience.

Qualifications

• Bachelor's degree in social science or related field; 2+ years experience serving adults with severe and persistent mental illness or in diversion services.

Responsibilities

• Serve as Primary Case Manager for assigned caseload, advocate for and coordinate client needs and services; ensure plans and documentation are current.
• Coordinate with internal/external departments to manage referrals, intake, and individualized plans.
• Collaborate with care team to ensure access to treatment, support, and appointments; arrange transportation as needed.
• Provide crisis intervention and ensure confidentiality; maintain trauma-informed interactions.
