Qualifications

• Bachelor's degree OR High School Diploma/Equivalence with 1 year of paid social services work experience.
• Valid Driver License and ability to drive an organization vehicle.
• Access to transportation travel locally.
• Act 33/34/73 Clearances
• Pre-Employment Drug Screen, Physical/TB
• Must pass Medication Training within 90 days of employment

Responsibilities

• Individualized Care Planning: Develop and implement individualized care plans with residents and families.
• Daily Living Assistance: Help with personal care, meal prep, medication management, and activities.
• Emotional and Behavioral Support: Provide empathetic support with de-escalation techniques.
• Community Integration: Organize social and recreational activities to promote belonging.
• Advocacy and Collaboration: Liaise with external agencies and healthcare teams for holistic care.
