Responsibilities

• Provides ongoing monitoring and interaction with consumers to include training on basic living skills (BLS) according to assessed needs.
• Meets daily productivity standards through individual and group BLS services.
• Assists consumers with personal care, medication use, chores, and safety issues.
• Prepares consumer meals according to dietary orders and menus.
• Transports consumers to appointments and outings.
• Documents in a timely fashion per AltaPointe policy.
• Treats consumers with care, dignity and compassion.
• Maintains house cleanliness and assists with housekeeping tasks.
• Performs hand-off communication at shift change and collaborates with the treatment team.
