Job summary

Responsibilities

• Provides ongoing monitoring and interaction with residents.
• Assists residents with personal care, meals, and daily living skills.
• Transports residents to appointments and outings.
• Documents services and progress notes in a timely manner.
• Attends staff meetings and collaborates with the treatment team.
