Job summary

Continuum of Care in New Haven, CT is seeking a dedicated caregiver for the 2nd shift (Sun-Sat? actually Sun-Thu 3:30pm-11:30pm) at $21.17 per hour with a $1 weekend differential.

The role assists individuals with daily routines, prompts for daily living skills, and leads group activities, while providing companionship and monitoring medications as needed. Responsibilities include documenting shift logs, maintaining a safe living environment, transporting clients to appointments, and

Qualifications

• High School Diploma or equivalent with minimum 1 year healthcare experience.
• Licensed CNA or related certification accepted in lieu of experience.
• Valid Connecticut driver's license with clean driving record.
• Proficient in Microsoft Office and electronic record keeping.

Responsibilities

• Assist with daily living activities including personal care, housekeeping, and nutrition.
• Lead psychoeducational groups and social activities.
• Provide transportation to appointments and plan travel.
• Document shift logs and reports in electronic records.
• Monitor medication self-administration and observe for safety.
• Respond to crisis situations per agency policy.
