Job summary

Continuum of Care is seeking caregivers for a 3rd shift position in New Haven, CT. The role includes hands-on support for daily living activities, medication monitoring, and documentation in electronic records.

The schedule runs Tuesday to Saturday from 11:30pm to 7:30am with a weekend differential of $1 per hour. Qualifications include a High School Diploma or equivalent, and a valid CNA or related certification is preferred.

Qualifications

• High School Diploma or equivalent
• Licensed CNA or related healthcare certification accepted in lieu of experience
• CPR/First Aid certification encouraged and may be provided during orientation
• Proficiency with Microsoft Office and electronic record keeping

Responsibilities

• Establish rapport and maintain professional boundaries with clients.
• Provide hands-on assistance for activities of daily living and teach independent living skills.
• Administer and monitor client medications as directed.
• Document shift logs and client information in electronic records.
• Attend staff meetings and supervision sessions; participate in crisis interventions as needed.
• Escort clients to appointments and assist with transportation and travel planning.
• Keep living spaces clean and safe; conduct rounds and safety checks.
