Catholic Charities of the Archdiocese of Chicago is seeking a Community Care Coordinator to conduct annual resident assessments, develop service plans, coordinate referrals, and support wellness programs across Cook and Lake counties.

You will collaborate with multidisciplinary teams, respond to resident concerns, monitor service delivery, and assist with housing placement while maintaining compassionate, privacy-conscious service.

Qualifications

• Minimum customer service experience with diverse populations.
• Ability to assess needs and develop resident service plans.
• Experience handling crises with sensitivity and discretion.
