Role Overview
The Director of Community Care Coordination for Affordable Housing provides strategic and operational leadership for service coordination across 19 affordable and supportive housing communities serving older adults throughout Chicago and surrounding suburbs. The role ensures residents have access to high-quality, person-centered services that promote housing stability, well-being, and the ability to age in place with dignity.
Minimum
Bachelor's Degree in Arts/Sciences (BA/BS) in a related field of study 1-3 years relevant experience
Strong knowledge of service coordination, supportive housing, behavioral health, and resident services, with the ability to lead programs, ensure compliance, analyze outcomes, build strategic partnerships, and drive operational excellence.
Excellent leadership, communication, collaboration, and problem-solving skills are required.
Master's degree in Arts/Sciences (MA/MS) in a related field of study 3-5 years relevant experience
Experience in affordable or supportive housing, healthcare, behavioral health, aging services, or community-based programs, with demonstrated success leading multi-site service delivery, implementing integrated care models, and fostering cross-sector partnerships that enhance resident outcomes and organizational impact.
Minimum: Obtain Mental Health First Aid certification within 6 months of hire. Obtain Professional Service Coordinator (PSC) certification or comparable affordable housing resident services credential within 18 months of hire.
Preferred: LSW, LCSW, Professional Service Coordinator (PSC), or equivalent resident services certification; training in trauma-informed care, crisis intervention, motivational interviewing, or aging services.
