Catholic Charities of the Archdiocese of Chicago is seeking a Director of Community Care Coordination for Affordable Housing to lead service coordination across 19 housing communities. The role focuses on housing stability, well-being, and aging in place with dignity, involving collaboration with health, behavioral health, and community providers.

Qualifications

Strong leadership and communication skills are required.
Ability to lead multi-site programs and coordinate services.
Experience in aging, housing, or health-related services is preferred.

Responsibilities

Oversee care coordination across 19 affordable housing communities; standardize policies and metrics.
Identify opportunities for program growth and quality improvement; analyze resident outcomes.
Partner with Property Management, Housing, Benefits, Finance, and other departments for coordinated service delivery.
Lead and develop the Community Care Coordination team with clear goals and feedback.
Develop staff through ongoing training and mentorship to strengthen engagement and impact.
