Catholic Charities of the Archdiocese of Chicago seeks a Care Coordinator to assess elderly and frail clients in community settings, develop care plans, and coordinate services across teams in Cook and Lake counties.
You will conduct timely assessments, maintain documentation, travel within assigned communities, and participate in training to ensure high-quality care. Bachelor's degree and IDOA certification required; starting wage is $23.30 per hour.

Role Overview
The Care Coordinator conducts timely assessments for elderly and frail clients in community settings, develops and maintains comprehensive care plans, provides ongoing case management and referrals, ensures accurate documentation and certifications, collaborates with internal teams, travels within assigned communities, and engages in continuous learning to support high‑quality client care.
