Responsibilities

Schedule and coordinate resident transportation for medical appointments, therapy, dialysis, and other off-site services.
Coordinate appointment times with residents, families, transportation providers, and medical offices.
Maintain an accurate transportation and appointment calendar.
Arrange transportation based on resident needs and appointment schedules.
Communicate appointment and transportation details to nursing and other departments.
Prepare residents and required paperwork for scheduled appointments.
Confirm transportation arrangements and follow up on changes or cancellations.
Maintain accurate records and assist with other administrative duties as needed.
Provide excellent customer service to residents, families, and healthcare providers.
