Job summary

PACS Group is seeking a qualified Social Worker to support residents and families within a healthcare setting. This position focuses on helping residents achieve the highest practicable level of self-care and well-being through comprehensive social services.

You will assess social and family information, develop care plans, provide emotional support, and assist with healthcare decisions and discharge planning, coordinating with community agencies as needed.

Qualifications

• Bachelor's Degree in Social Work or Human Services required.
• 2 years of supervised social work experience in a health care setting preferred (MSW preferred).
• Strong communication, planning, and counseling skills.

Responsibilities

• Establishes course of action by exploring options; setting goals with resident and their families.
• Assist residents in achieving the highest practicable level of self-care, independence and well-being.
• Provide medically related social services to attain or maintain highest practicable physical, mental and psychosocial wellbeing.
• Evaluate social and family information to assist in social services needs and develop care plans.
• Provide emotional support and address emotional problems for residents and families.
• Assist residents with health care decisions and discharge planning.
• Coordinate with community and social services to meet residents' needs.
• Develop relationships with other departments and external agencies to ensure social services meet resident needs.
• Document resident social service status and assist with outpatient appointments and on-site services.

Education

Bachelor's Degree in Social Work or in Human Services and 2 years of supervised social work experience in a health care setting
