Job summary

The role involves working with administration, medical and nursing staff to ensure residents' psychosocial needs are met and maintained.

The candidate will help develop policies, coordinate care with other departments, assist families with resources, and document progress.

Qualifications

• Licensure as social worker per state requirements.
• Experience in a health care setting such as hospital or nursing facility.
• Ability to communicate effectively with residents and families.

Responsibilities

• Assist in planning, developing, and directing social service programs.
• Meet with administration and staff to plan social services.
• Develop and coordinate department policies and procedures.
• Review policies and participate in changes annually.
• Provide information on Medicare/Medicaid and financial assistance.
• Record progress notes and coordinate with other departments.
