PACS Group is seeking a Social Service Worker to assist in planning, developing, and directing social service programs for residents in a healthcare facility. You will collaborate with administration, medical staff, and families to meet psychosocial needs and support discharge planning.

The role requires a Bachelor's in Social Work (BSW) or equivalent in a health care setting, with experience and knowledge of federal/state regulations.

Formación

• Bachelor’s degree in social work required; or a related human services field with supervised experience.
• Ability to interpret and apply federal/state regulations and facility policies.
• Strong communication and documentation skills are essential.

Responsabilidades

• Assist the Social Service Director in planning, developing, implementing, and evaluating social services.
• Coordinate with administration, medical, and nursing staff to meet residents' psychosocial needs.
• Provide information to residents/families about Medicare/Medicaid and financial assistance.
• Maintain progress notes and participate in discharge planning and community resource referrals.
