Job summary

PACS Group in the United States is seeking a Social Services Coordinator to assist in planning, developing, organizing, implementing, evaluating, and directing our facility's social service programs in alignment with federal, state, and local standards.

You will collaborate with administration and medical staff, support discharge planning, coordinate resources, provide guidance on benefits, and maintain progress notes to help residents achieve the highest practicable level of well-being.

Qualifications

• High school diploma or equivalent; associate or bachelor’s degree preferred.
• Experience in social services and coordination of care is desirable.
• Ability to document and communicate clearly with residents, families, and staff.

Responsibilities

• Assist in planning, developing, organizing, implementing, evaluating, and directing social service programs.
• Meet with administration and staff to plan and coordinate services and policies.
• Provide information on Medicare/Medicaid and other financial assistance programs to residents and families.
• Maintain regular progress notes and coordinate discharge planning with community agencies.
• Assist residents in achieving the highest practicable level of self-care and well-being.
