Job summary

Qualifications

• Bachelor’s Degree in Social Work or Human Services and 2 years of supervised social work experience in a health care setting.
• MSW preferred.

Responsibilities

• Establishes course of action by exploring options; setting goals with resident and their families.
• Assist residents in achieving the highest practicable level of self-care, independence and well-being.
• Provide medically related social services to attain or maintain the highest practicable wellbeing.
• Evaluate social and family information to assist in care plans for social services issues.
• Provide emotional support addressing issues related to coping with disabilities and health fears.
• Assist residents with health care decisions and discharge planning with appropriate agencies.
• Develop and maintain relationships with departments and outside agencies to meet resident needs.
• Document resident social service status and coordinate outpatient services as ordered.
• Counselling residents and family members.
