Job summary

PACS is seeking a dedicated Social Worker to deliver social services within a post-acute healthcare setting. You will support residents and families in goal setting, care planning, and coordinating services to enhance wellbeing.

The role requires a Bachelor’s in Social Work or Human Services, with MSW preferred, and at least two years of supervised healthcare social work experience. On-site position with collaboration across departments and community agencies.

Qualifications

• Bachelor’s Degree in Social Work or in Human Services; MSW preferred.
• At least 2 years of supervised social work experience in a health care setting.
• Ability to read and interpret policy and procedure manuals.

Responsibilities

• Help residents and families set goals and plan care with families.
• Assist residents to achieve self-care and independence.
• Provide social services to enhance physical, mental, and psychosocial wellbeing.
• Develop care plans and coordinate with interdisciplinary teams.
• Arrange outpatient appointments and coordinate community resources.
