Responsabilidades

• Assist residents in achieving the highest practicable level of self-care, independence and well-being.
• Provide medically related social services to support physical, mental, and psychosocial wellbeing.
• Evaluate social and family information to develop care plans for social services issues.
• Provide emotional support to residents and families and assist with coping related to health conditions.
• Assist with discharge planning and outpatient appointments as required.
• Develop and maintain relationships with other departments and community agencies to deliver social services.
