Job summary

Community Housing Innovations, Inc. is seeking a Social Worker to join a human service team. You will develop and execute service and care plans for residents, promoting stabilization, independence and skills development in a supportive environment.

The role involves assessing needs, providing direct services, coordinating with external providers, and supporting residents with health, housing and employment goals. Schedule is Monday‑Friday, 8am‑4pm, in East Chestester Bronx, NY.

Qualifications

• Master’s Degree in Social Work; LMSW preferred.
• Three (3) years of experience in case management, counseling or direct advocacy.
• Experience with immigrants and immigration issues prioritized.

Responsibilities

• Develop and implement service and care plans for residents to foster stabilization and independence.
• Assess resident needs and provide direct services.
• Bridge residents to external service providers and promote healthy behaviors.
• Coordinate workshops and activities to enhance self-care and adjustment.
• Collaborate with external providers for mental health services including trauma recovery.
• Meet residents weekly to review progress and adjust plans.
• Coordinate discharge plans and ensure service linkages.
