Qualifications

Responsibilities

• Assess psychosocial functioning and contribute to support plans with clients and team.
• Conduct psychosocial assessments and provide treatment across diverse backgrounds.
• Coordinate with medical and mental health providers, home care agencies, and community organizations.
• Maintain timely documentation in compliance with agency and funder requirements.
