Job summary

Community Housing Innovations (CHI) in NYC Bronx seeks a Social Worker to develop and execute service and care plans for residents, fostering stabilization, independence, and skill development.

You will assess resident needs, provide direct services, coordinate with case managers, and connect residents to external resources, including health, housing, and employment supports. Schedule: Mon–Fri, 8am–4pm; Salary: $66,300 annually.

Qualifications

• Master’s Degree in Social Work required; LMSW preferred.
• Excellent verbal and written communication skills in English; multilingual and Spanish proficiency prioritized.
• Computer skills: MS Word, Excel, Outlook; familiarity with CARES, HOME, CAPS and AWARDS.

Responsibilities

• Develop and facilitate service and care plans for residents to support stabilization, independence and skills development.
• Assess resident needs, provide direct services and bridge residents to external service providers.
• Meet weekly with caseload residents to monitor progress and adjust plans; coordinate discharge plans and referrals.
