Job summary

Bronx Family Network Inc. is seeking a Social Worker to provide case management, counseling, and resource coordination for shelter clients in Queens, NY.

The role focuses on needs assessments, service planning, and connecting clients with housing opportunities and DHS-supported services. Responsibilities include group sessions, interagency coordination, data entry in CARES, and supporting residents with trauma-informed, client-centered care.

Qualifications

• Master’s degree in social work from an accredited institution.
• CSW or LMSW license from NYS Education Department required within 1 year of appointment.
• Familiarity with local housing resources, public benefits, and community-based services.

Responsibilities

• Provide case management, counseling, and resource coordination for shelter clients.
• Conduct needs assessments and develop individualized service plans.
• Coordinate services and monitor progress to move clients from shelter to permanent housing.
• Conduct or participate in group sessions and interagency case conferences as needed.
• Maintain case notes and data in DHS/CARES database per standards.
