Job summary

Responsibilities

• Provide case management, resource navigation, and referral services to residents.
• Connect residents with appropriate community agencies, social services, and support programs.
• Coordinate resident services, educational opportunities, and community resources.
• Maintain accurate case notes, resident files, referrals, follow-ups, and HUD-required documentation.
• Collaborate with on-site management, volunteers, and community partners.
• Maintain confidentiality of resident information and follow HIPAA requirements.
• Follow mandated reporting requirements and organizational policies.
• Work collaboratively with the team to support positive resident outcomes.
