Hill Valley Healthcare in Nashville, TN is seeking a Social Services Director to lead the social, emotional, and psychosocial support for residents, ensuring compassionate care and compliance with regulations.

You will advocate for residents, coordinate care with families and staff, and oversee documentation, discharge planning, and care planning committees, while promoting a warm, team-oriented culture.

Qualifications

• Bachelor's degree in Social Work, Human Services, or a related field preferred.
• Strong communication, organizational, and interpersonal skills.
• Ability to advocate effectively for residents while maintaining professionalism and confidentiality.

Responsabilités

• Oversee and coordinate the social services program to ensure residents receive appropriate support and resources.
• Advocate for residents and assist with addressing their individual needs, concerns, and preferences.
• Complete and maintain accurate social services documentation and assessments in accordance with applicable regulations and facility policies.
• Assist residents and families with adjustment to the facility and provide support throughout the resident’s stay.
• Coordinate care and communicate effectively with residents, families, staff, and outside service providers.
• Participate in care planning and interdisciplinary team meetings.
• Assist with discharge planning and coordinate appropriate community resources and services.
• Maintain compliance with federal, state, and local regulations related to social services and long-term care.
