Qualifications

• Bachelor's degree in Social Work is required; MSW preferred.
• Minimum 2 years in long-term care or healthcare; supervisory experience preferred.
• Knowledge of MDS, care planning, and discharge planning.
• Strong communication, organizational, and leadership skills.
• Licensed Social Worker in PA preferred, but not always required depending on facility policy.

Responsibilities

• Oversee the Social Services department and ensure regulatory compliance.
• Conduct psychosocial assessments and develop individualized care plans.
• Support residents and families through admissions, discharges, and transitions of care.
• Coordinate services with interdisciplinary team members and outside agencies.
