Qualifications

• Bachelor's degree in Social Work, Human Services, Psychology, or a related field required.
• Master's degree in Social Work preferred.
• Current state licensure or certification, if required by state regulations.
• Minimum of two years of social services experience in a Skilled Nursing Facility, Rehabilitation Center, Long-Term Care, or healthcare setting preferred.
• Previous supervisory or management experience preferred.
• Strong knowledge of discharge planning, care coordination, and long-term care regulations.
• Excellent communication, organizational, and interpersonal skills.
• Ability to manage multiple priorities in a fast-paced environment.

Responsibilities

• Direct and oversee all functions of the Social Services Department.
• Complete psychosocial assessments and develop individualized care plans.
• Coordinate discharge planning and facilitate safe transitions of care.
• Provide counseling and emotional support to residents and families.
• Assist residents and families with advance directives, healthcare proxies, and community resources.
• Participate in interdisciplinary care plan meetings and quality improvement initiatives.
• Advocate for resident rights and promote resident satisfaction.
• Ensure compliance with federal, state, and local regulations governing social services in long-term care.
• Maintain accurate documentation and required assessments.
• Investigate and address resident concerns, grievances, and family issues.
• Collaborate with nursing, rehabilitation, admissions, and other departments to support resident-centered care.
• Prepare for and participate in state and federal surveys.
