Job summary

Healthcare Nursing Center seeks a dedicated Social Service Director to oversee the Social Services department and provide emotional support, counseling, and resource linkage for patients and families.

You will work with an interdisciplinary team to create individualized care plans that address physical, emotional, and social needs, while guiding discharge planning and staff mentorship for high-quality care outcomes.

• Oversee Social Services department to provide emotional support and linkage to community resources.
• Collaborate with interdisciplinary team to develop care plans.

Responsibilities

• Develop and implement individualized care plans in collaboration with patients, families, and the interdisciplinary team
• Provide emotional support and counseling to patients and families
• Connect patients with community resources such as home healthcare and long-term care facilities
• Coordinate discharge planning and ensure smooth transition home
• Supervise and mentor Social Workers and support staff
• Participate in quality improvement initiatives to improve patient outcomes and satisfaction
• Maintain accurate records of patient care and services provided
