Qualifikationen

• Bachelor's degree in Social Work or related field is required.
• LCSW licensure preferred.
• Experience working with older adults is required or strongly preferred.
• Proficient with Microsoft 365 (Excel, OneDrive, Outlook, Word).
• Technical aptitude and ability to learn Resident Portal and EMR systems.

• Identify residents' emotional, physical, spiritual, and social needs and connect them with appropriate resources.
• Oversee smooth transitions for new residents and coordinate orientation.
• Monitor changes in resident condition to determine level of care placements.
• Access and review EMR system data to coordinate care and services.
• Document all interactions in resident records and with families.
