Qualifications

• Bachelor’s degree or higher in Social Work or related field.
• Experience in senior living, healthcare, or resident services preferred.
• Strong communication, problem-solving, and interpersonal skills.

Responsibilities

• Assess residents’ social, wellness, and healthcare needs.
• Support residents and families during transitions across levels of care.
• Collaborate with interdisciplinary teams and attend care coordination meetings.
• Follow up with residents after hospitalization or significant health events.
• Maintain accurate and confidential documentation.
• Promote internal services and wellness programs.
• Develop and maintain relationships with internal and external resources.
