Job summary

Qualifications

• Bachelor’s degree in Social Work, Psychology, Counseling, or related field preferred.
• Minimum 2 years of experience providing social services.
• Working knowledge of community resources, supportive services, entitlement programs, and social service systems.
• Proficiency with Microsoft Office.
• Previous experience with HUD regulations or affordable housing programs is a plus.

Responsibilities

• Assess residents’ psychological, physical, social, and service needs and provide referrals.
• Coordinate supportive services to help older adults maintain independence and age in place.
• Connect residents with community resources and entitlement programs including transportation, meals, home health services, counseling and financial assistance.
• Develop and maintain relationships with community agencies, healthcare providers, social service organizations, and other service partners.
• Advocate for residents and assist them in navigating benefits and support services.
• Organize educational programs, wellness initiatives, social activities, and community events.
