Summary

The Housing Coordinator, under the Money Follows the Person (MFP) program, works with consumers, families, providers, landlords, property owners, and other staff to coordinate, manage, and implement housing tools, resources, and inventories to ensure that consumers secure safe and appropriate housing. At CCC all duties are performed in a manner that fosters the achievement of the organization's mission.

Essential Functions
  • Works with realtors, landlords, and others to identify potential units in subsidized public and/or private housing, assuring MFP clients can be presented with at least 3 options.
  • Coordinates with realtors and landlords within geographic area assigned by the department.
  • Assists clients with arranging transportation to view units and/or provides clients with adequate visual representations of available units from which to choose.
  • Completes and submits required housing paperwork.
  • Identifies the need for modifications to a rental unit and follows protocols.
  • Identifies and gains landlord approval regarding accessibility modifications required to make housing accessible and safe for clients.
  • Ensures that consumers are aware of their role and responsibilities regarding applicable subsidies.
  • Assist clients after transition with issues pertaining to rental payments, landlord relationships, and local housing authorities.
  • Maintain up-to-date and accurate documentation in the state system.
  • Attends company and state meetings as required.
  • Performs other related projects or assignments as requested including photocopy work, data entry, typing, distribution of reports, etc.
Requirements
Education

High School Diploma or equivalent; Bachelor’s Degree in Human Services or related field preferred.

Experience
  • One to two years experience in housing service coordination for the aging and persons with disabilities is preferred.
  • Experience in real estate market and negotiation a plus.
  • Knowledge of the Independent Living philosophy a plus.
  • Knowledge of community and housing resources in Connecticut, with the ability to understand and articulate multiple programs.
  • Proficient computer skills, including Microsoft Office.
  • Strong communication skills, both written and verbal.
  • Ability to keep organized and work effectively in a fast-paced, dynamic environment.
  • Familiarity with the needs of multiple vulnerable populations, especially older adults and persons with disabilities.

Reliable transportation, valid driver’s license and current automobile insurance.
