A leading property management organization is seeking a Resident Service Coordinator (RSC) to support an elderly and disabled housing community in West Hartford, CT.
The Resident Service Coordinator helps residents identify and access services, benefits, and community resources that promote independence, stability, and successful aging in place. The RSC works closely with the Property Manager and residents to assess needs, coordinate supportive services, advocate on behalf of residents, and build a strong sense of community.
The ideal candidate is compassionate, organized, resourceful, and comfortable working independently while collaborating with residents, families, property management, healthcare providers, and community organizations.
Key Responsibilities
- Assess residents’ psychological, physical, social, and service needs and provide appropriate referrals and follow-up support.
- Coordinate supportive services that help older adults and individuals with disabilities maintain independence and age in place.
- Connect residents with community resources and entitlement programs, including transportation, meals, home health services, counseling, financial assistance, and healthcare resources.
- Develop and maintain relationships with community agencies, healthcare providers, social service organizations, and other service partners.
- Advocate for residents and assist them in navigating available benefits, programs, and support services.
- Organize and facilitate educational programs, wellness initiatives, social activities, and community events.
- Assist residents and families with care planning and transitions to higher levels of care, including nursing home placement when appropriate.
- Partner with property management to address resident concerns, assist with conflict resolution, and respond to unusual resident behavior or issues identified during inspections and recertifications.
- Maintain accurate and confidential resident service records, case notes, releases, and supporting documentation.
- Prepare monthly and annual reports and maintain documentation required by management, owners, and HUD.
- Communicate regularly with residents through newsletters, flyers, calendars, meetings, and other community communications.
- Utilize AASC and other industry software to document services, track resident needs, and manage reporting requirements.
- Attend required training programs and perform additional administrative or resident service responsibilities as assigned.
Qualifications
- Bachelor’s degree in Social Work, Psychology, Counseling, or a related field preferred.
- Minimum 2 years of experience providing social services, preferably working with elderly and/or disabled populations.
- Working knowledge of community resources, supportive services, entitlement programs, and social service systems.
- Strong advocacy, organization, communication, and problem-solving skills.
- Demonstrated ability to assess resident needs, coordinate services, and follow through to achieve results.
- Strong interpersonal skills and the ability to establish trusting relationships with residents and families.
- Proficiency with Microsoft Office required.
- Previous experience with HUD regulations or affordable housing programs is a plus.
- Ability to maintain confidentiality and handle sensitive resident information professionally.
