Job summary

National Church Residences is seeking a Service Coordinator I to coordinate aging-in-place programs and connect residents with community services. You will act as a liaison to providers, organize health-related events, and ensure timely documentation of services and outcomes.

The role emphasizes a person-centered approach, adherence to program requirements, and collaboration with care teams to support residents in maintaining quality of life in a housing setting.

Qualifications

• High School Diploma or GED required; a BA/BS is preferred for advanced roles.
• Two+ years in social services with elderly, disabled, or family populations.
• Strong communication and interpersonal abilities; proficient with basic computer tools.

Responsibilities

• Coordinate programs and activities for eligible individuals.
• Liaise with community agencies; build partnerships and negotiate discounts.
• Document services in the management system and monitor provision of support.
• Develop and maintain a directory of local service providers.
• Organize educational events related to health care, life skills and referrals.
• Assist participants in building informal support networks.
• Maintain documentation and follow up according to program requirements.
• Contribute to grant activities and special programs benefiting participants.
