Family Service Association of Bucks County in Langhorne, PA is seeking compassionate Case Managers to join our team. We support individuals and families facing housing instability, homelessness, and related challenges, offering person-centered advocacy and coordinated services.

These full-time roles involve housing access, service plan development, progress monitoring, and collaboration with community partners, with a focus on trauma-informed and Housing First approaches.

Qualifications

• Bachelor’s degree in Social Work or related field, or equivalent combination of education and experience.
• Valid driver’s license required.
• Experience in case management or homelessness services preferred.
• Proficiency with HMIS, Credible, ADP and MS Office.

Responsibilities

• Provide case management, advocacy, and coordination of services for individuals experiencing homelessness.
• Assist clients in accessing housing and community supports.
• Develop individualized service plans and monitor progress.
• Collaborate with internal teams and community partners.
• Ensure documentation compliance per funder and agency standards.
• Apply trauma-informed, Housing First, and harm reduction approaches.
