BHRAGS ALLIANCE is seeking a Case Manager in Brooklyn to conduct intake, assess residents’ needs, and develop individualized service plans. You will monitor progress, coordinate with shelter staff, and advocate for access to housing and social services.

The role requires at least a 2-year degree in Human Services or related field, counseling experience, and strong communication skills. Bilingual English/Spanish is preferred. Full-time position with hourly pay.

Responsabilités

• Conduct intake and assess residents’ needs.
• Develop individualized service plans with short and long-term goals.
• Monitor progress with weekly contacts and updates.
• Maintain organized filing systems for residents.
• Collaborate with shelter staff to enforce rules and regulations.
• Advocate with agencies to minimize barriers to services.
• Follow up with referrals to ensure timely access to services.
• Coach clients in job prep, housing services, and social services.
• Represent BHRAGS ALLIANCE at coalitions and committees as needed.
• Participate in staff meetings and case conferences as directed.
