S:US is seeking a Case Manager for their men's shelter in Brooklyn. This full-time role involves conducting psychosocial, vocational, and housing assessments, documenting progress, and assisting clients with housing applications toward permanent affordable housing.

Salary ranges USD 45,000 to 51,376 per year. The position requires a Bachelor's Degree in Human Services (or HS/GED with relevant experience), experience with chronically homeless individuals, and the ability to work with a culturally

Qualifications

• Bachelor’s Degree in Human Services preferred; HS diploma or GED required
• Experience working with chronically homeless individuals with co-occurring disorders is desirable
• Ability to work with a culturally diverse population; bilingual preferred; NYS Driver’s License preferred but not required

Responsabilités

• Conduct psychosocial, vocational, and housing assessments with documentation
• Provide supportive social services and track client progression toward housing goals
• Prepare housing applications and assist clients with the housing interview process
• Monitor client progress and coordinate with nursing staff on medications and emergencies
• Facilitate group and individual counseling, and coordinate visits and inspections
• Attend meetings, case conferences, and housing interviews as needed
