Responsibilities

• Conduct intake within 24 hours of admission and explain resident rights and procedures.
• Document psychosocial, housing, and vocational assessments.
• Provide accurate, complete documentation of services and progress toward housing.
• Assist with housing applications and review progress with applicants.
• Coordinate with medical and community services; provide crisis intervention.
• Maintain client records per DHS/OTDA standards.
