Job summary

Oakwood Gardens Care Center is seeking a compassionate Social Services Assistant to join our interdisciplinary team. The role supports care planning, discharge coordination, resident advocacy, and communication with families, under the Social Services Director.

Excellent organizational and documentation skills are essential, along with the ability to work with residents and families through medically related social needs. Competitive hourly pay and full-time benefits are included.

Qualifications

• High school diploma or GED is required.
• Strong written and verbal communication skills.
• Ability to maintain confidentiality and professional boundaries.
• Proficiency with basic computer applications and standard office equipment.

Responsibilities

• Assist the Social Services Director in planning, developing, implementing, and evaluating the facility’s social services programs.
• Help identify and address residents’ medically related social, emotional, and psychosocial needs.
• Support residents in achieving their highest practicable level of independence and well-being.
• Assist residents and families with concerns related to illness, admission, disability, loss, anxiety, stress, and adjustment to facility life.
• Gather social and family information and assist with developing individualized social services plans.
• Complete required forms, reports, assessments, progress notes, and other documentation.
