Oak Hill Senior Living is seeking a full-time Social Service Assistant to support the Social Services Director in delivering resident-centered care. You will help assess social needs, coordinate services, and maintain sensitive resident information with confidentiality.

The role involves assisting with interviewing residents and families, arranging transportation, and monitoring mood and behaviors to support individualized care plans.

Qualifications

• Must possess a high school diploma or GED.
• Have a thorough understanding of the principles of best Social Services practices.

Responsabilités

• Assist the Social Services Director in operating the Social Services Department.
• Interview residents/families to obtain social history.
• Arrange transportation to other facilities when necessary.
• Monitor resident mood/behaviors and assist in care planning.
• Assist in developing and implementing behavior management programs.
• Maintain and obtain eyewear, hearing aids, dentures and personal items for residents.
• Assist with psychotherapeutic medications oversight and care plan documentation.
• Document care plans, CQI and MDS data, and progress notes.
• Schedule and document care plan meetings and ensure notes are followed.
