Job summary

Oakwood Gardens Care Center is seeking a compassionate and organized Social Services Assistant to join our interdisciplinary team as a full-time member. Under the Social Services Director, you will help address residents’ medically related emotional and psychosocial needs.

You will support care planning, discharge coordination, resident advocacy, and coordination with families and community resources. A strong emphasis on confidentiality and professional communication is essential.

Qualifications

• High school diploma or GED required.
• Associate or bachelor’s degree in Social Work, Human Services, Psychology, Sociology, or a related field preferred but not required.
• Experience in healthcare, skilled nursing, long-term care, social services, or customer-service preferred.

Responsibilities

• Assist Social Services Director with planning, developing, implementing, and evaluating social services programs.
• Support residents in achieving independence and well-being.
• Assist residents and families with illness, admission, disability, loss, anxiety, stress, and adjustment to facility life.
• Complete forms, assessments, progress notes, and documentation; maintain progress notes.
• Participate in care conferences, interdisciplinary meetings, and discharge-planning discussions.
• Coordinate social services with other departments and community resources.
