Job summary

FutureCare is seeking a Physical Therapist in Maryland to evaluate and treat movement dysfunction, restore mobility, and develop individualized care plans for residents in a long-term care setting.

Qualifications

  • Bachelor of Science degree in Physical Therapy.
  • Current license or eligibility for licensure in Maryland.
  • Minimum 1 year of Long Term Care experience preferred.
  • Certification and demonstrated competency in identified areas of specialization.
  • Demonstrated competency in applying the principles, methods, materials and equipment used in physical therapy.
  • Ability to assess resident needs and develop and implement a comprehensive plan of care.
  • Knowledge of accreditation standards and compliance requirements.

Responsibilities

  • Review available resident information and evaluate the impact on assessment and treatment.
  • Conduct and document a thorough functional assessment of the resident, including home/vocational and equipment needs.
  • Develop and implement an individualized treatment plan based on resident needs with achievable functional goals.
  • Comply with evaluation, treatment and documentation guidelines per facility policy and regulations.
  • Complete re-evaluations according to practice regulations and review interventions by assistants as appropriate.
  • Instruct assistants and rehab techs in treatment procedures and provide supervision.
  • Document the course of treatment in the resident's medical record following each session.
  • Revise treatment plans as needed throughout the treatment process.
  • Document discharge summaries in a clear, concise format.
  • Orient residents to program services and inform them of potential risks of procedures.
  • Collaborate with nursing staff, residents and families to maximize functional abilities.
  • Educate residents and family about exercise programs to be continued after discharge.
  • Assist in referrals for additional services after discharge and stay informed about community resources.
