FutureCare Health & Management is seeking a licensed Physical Therapist in Maryland to evaluate and treat movement dysfunction, restore mobility, and develop individualized plans for residents’ independence and well-being. The role emphasizes compassionate care, collaboration with nursing staff and families, thorough documentation, and ongoing learning within a supportive team.

Qualifications

• Bachelor degree in Physical Therapy.
• Current license or Maryland licensure eligibility.
• 1 year LTC experience preferred, new grads welcome.
• Certification in specialization areas or demonstrated competency.
• Ability to apply PT principles, methods and equipment.
• Ability to assess needs and develop a plan of care.
• Knowledge of accreditation standards and compliance.

Responsabilités

• Review resident information and assess impact on treatment.
• Conduct functional assessments using standardized procedures.
• Develop and implement individualized treatment plans with goals.
• Comply with facility policies and state/federal regulations.
• Complete re-evaluations as required.
• Instruct and supervise rehab staff during sessions.
• Document care in the resident's medical record after each session.
• Revise treatment plans as needed throughout care.
• Document discharge summaries using prescribed format.
• Orient residents to services and inform of risks.
• Collaborate with nursing staff and families to maximize function.
• Educate residents and families on home exercise programs.
• Assist in referrals after discharge and know community resources.
