Job summary

FutureCare Health & Management in Maryland is seeking a Physical Therapist to assess and treat residents, develop individualized plans, and ensure high-quality care in a long-term care setting.

The role requires a Bachelor of Science in Physical Therapy and a current Maryland license or eligibility. Experience in long-term care is preferred, with opportunities to supervise staff and contribute to discharge planning. Competitive pay and benefits are offered.

Qualifications

• Knowledge of standard PT assessment tools and documentation procedures.

Responsibilities

• Review available resident information and evaluate impact on assessment.
• Conduct and document thorough functional assessment using standardized procedures.
• Develop and implement individualized treatment plans with achievable goals.
• Comply with facility policies, practice regulations, and state/federal rules.
• Complete re-evaluations per regulations and oversee PT assistants when applicable.
• Instruct and supervise rehab staff during treatments.
• Document treatment sessions clearly in the resident’s medical record.
• Revise treatment plans as needed throughout care.
• Prepare discharge summaries using prescribed formats.
